Overview

Impaired verbal communication is a condition among patients in which there is an inability to produce a word or sentences, having difficulty to communicate or speak and or lastly, the thoughts being expressed are a challenge for the patient. There are several nursing care plan that can be used for patients with such condition, both men and women. First, the nurse must assess and find the type or root cause of the problem. Second, the nurse must be able to teach and help the client about several ways for them to communicate and share their immediate needs. First, the nurse must teach the patient about the use of call bell whenever there are problems. Secondly, the nurse can give the patient (if not impaired) some pen and paper to write his or her concerns. If the patient is a stroke patient, the nurse can provide other tools by showing word cards such as a food when the client wants to eat for example. When there is a tracheostomy, the nurse can teach the patient on how to speak lit tle by little. Lastly, the patient can be refer red to a speech pathologist to help the patient speak with the help of several interventions.

Speech impairment can be a difficult time for both the patient (most especially if it's a child) and the health care worker. The nursing staff must be patient in dealing with them.
