The nursing staff must address all the health needs of a stroke patient from admission to discharge to home. An excellent nursing care plan must be implemented all throughout. From admission, the nurse must quickly assess the patient and prepare for diagnostic procedures or exams such as CT-Scan or MRI with contrast. If the bleeding is massive, the patient can be referred to a neurosurgeon for intervention. If it is manageable by drugs, the neurologist can do the management and treatment. The patient is usually placed in ICU for monitoring. If all is well, nurses can now do the plans for discharge. Besides health education about healthy lifestyle, the nurse can also help the patient by referring the patient to rehabilitation medicine per doctor's order.
